LIABILITY WAIVER AND RELEASE

As the parent or legal guardian of the below named minor child, acknowledge and agree to the following:

ASSUMPTION OF RISK

I understand that participation in THE CREW Summer Camp activities involves inherent risks, including but not limited to:

• Use of art materials, tools, and crafting supplies

• Physical activities and movement within the facility

• Interaction with other children and staff

• Potential for cuts, bruises, or other minor injuries

• Allergic reactions to materials (despite precautions)

• Risk of property damage or loss

Understanding these risks and others not specifically listed, I voluntarily assume all risks associated with my child’s/children’s participation in THE CREW Summer Camp.

RELEASE OF LIABILITY

I hereby release, waive, discharge, and hold harmless THE EXCHANGE, their owners, operators, employees, volunteers, agents, and representatives (collectively “Released Parties”) from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury (including death) that may be sustained by my child/children or property while participating in camp activities, whether caused by the negligence of the Released Parties or otherwise.

MEDICAL AUTHORIZATION

I authorize THE EXCHANGE staff to seek emergency medical treatment for my child/children if needed. I understand that I will be contacted immediately, but consent to necessary medical care if I cannot be reached. I agree to be responsible for all medical expenses incurred.

BEHAVIORAL EXPECTATIONS

I understand that THE EXCHANGE maintains behavioral standards to ensure a safe environment. THE EXCHANGE reserves the right to dismiss any participants whose behavior disrupt the program or endanger others, without refund.

ADDITIONAL PROVISIONS

• This waiver shall be binding upon my heirs, assigns, and legal representatives

• If any provision is deemed unenforceable, the remainder shall remain in effect

• This agreement is governed by California law

• Any legal action must be brought in Solano County, California
